Keep Complex Cases in Your Office — We Bring the Sedation to You

Some of your patients need IV sedation to get the care you've already diagnosed. Referring them out means a case leaves your practice and sometimes never comes back.

We come to you instead. Dr. Michael Schaake is an IV sedation trained general dentist who holds a level 3 sedation permit and ASA 3 endorsement with the TSBDE. He travels to your office with the equipment, monitoring, and clinical experience to deliver moderate IV sedation for your patients in your operatory, while you perform the dentistry. What mobile IV sedation is

Mobile IV sedation - also called mobile dental anesthesia - is a visiting sedation provider who brings sedation services on-site to a host dental practice. Dr. Schaake arrives with the sedation medications, monitoring equipment, and emergency drugs and airway kit, establishes and manages the IV, monitors the patient throughout the procedure, and stays until the patient meets discharge criteria.

You keep doing what you do best. We handle the patient's comfort and their vitals from start to finish.

Rapid Relaxation: The effects of IV sedation are almost immediate, helping the patient feel calm within minutes.

Adjustable Sedation Level: We can customize the amount of sedation to fit your needs, ensuring you stay comfortable throughout the procedure.

Comfortable Experience: Many patients report little to no memory of the procedure afterward, making future visits less intimidating.

Cases We Commonly Support:

Moderate IV sedation can be a great option for various use-cases

Full Mouth Rehabilitation and full-arch implant cases.

Multiple and/or surgical extractions, including 3rd molars.

Long or Complex Treatments: For patients needing extended or multiple treatments in one appointment, sedation can make the time pass more comfortably.

Long restorative appointments

Patients with severe dental anxiety or dental phobia

Patients with a strong gag reflex

Patients who have had difficulty achieving profound local anesthesia

How a sedation day works

We want to make the process as seamless and smooth for your office and patient as possible. This is how the process works:

Scheduling and Case Review
Call our office to schedule the case and send us the patient's medical history, medication list, and planned treatment. We review for sedation candidacy and contact the patient for a remote consult and collect payment from the patient directly.

Pre and Post Operative Instructions
We provide written and verbal pre and post-operative instructions - fasting, medication changes, and the care-giver contract. We also bring a wheel chair to escort patient to their care-giver's vehicle.

The Appointment
Dr. Schaake arrives ahead of the scheduled start, sets up in your operatory, and reviews the patient's history and vitals. He starts the IV, titrates the patient to a moderate level of sedation, administers local anesthetic and monitors continuously using pulse oximetry, blood pressure, ECG and capnography for the entire procedure as well as completing a board-compliant sedation record you can add to your chart. You perform the dentistry.

Post-Treatment Care and Recovery
Recovery and discharge. We stay with the patient through recovery, confirm they meet discharge criteria, and release them to their caregiver with written post-operative instructions. Dr. Schaake also does a next-day call to the patient, plus a written sedation record you can add to your chart.

Why not just Refer out? Advantages of in-house IV sedation

IV sedation in your office minimizes the number of trips that patients need to take to specialist's offices just because they need sedation. Often, you are able to do the procedure, but the patient cannot tolerate it without sedation. Many patients are very appreciative of being able to keep the sedation in-house.

You keep the case. Full-mouth rehab, multiple extractions, implant placement, and long restorative appointments stay in your practice instead of going to an outside specialist.

Treatment actually gets completed. Fearful patients who have deferred care for years will often say yes to sedation when they won't say yes to another appointment.

Fewer, longer visits. Work that would take four appointments can often be consolidated into one sedated visit - better for your schedule and for the patient.

No capital outlay. No sedation permit, no monitors, no emergency drug inventory, no continuing-education burden on your team.

Your patients stay in a familiar office with the dentist they already trust.

Is IV Sedation Safe?

Dr. Michael Schaake holds a Level 3 Sedation Permit in Texas, having received his training through the Academy of Dental and Medical Anesthesia and is ACLS certified. He has 2 years of experience providing IV sedation and 4 years of experience providing oral conscious and nitrous oxide sedation.

Dr. Schaake routinely sedates and treats patients in his own practice with moderate IV sedation and stays current on topics in sedation dentistry and pharmacology.

Every patient is screened for sedation candidacy before the appointment via a phone consultation with Dr. Schaake, monitored continuously during it, and discharged only after meeting objective recovery criteria.

Common questions from dentists

Q: Do I need a sedation permit to host you?
A: No. Dr. Schaake's sedation permit covers the sedation.

Q: Who is responsible for the patient?
A: Dr. Schaake is responsible for the sedation and for the patient's airway and vitals. You remain responsible for the dental treatment you perform.

Q: How far in advance do I need to schedule?
Availability varies. Please reach out as soon as the patient has indicated they are ready to move forward with treatment to reserve your appointment time. From there, we will reach out to the patient to complete the phone consultation and payment.

Q: How is it billed?
We require payment up front from the patient for the procedure. Rescheduling or Cancellations within 72 hours will be refunded at 50% of the total fee. Cancellations within 72 hours will not be refunded

Q: Who will administer the local anesthetic?

A: Dr. Schaake prefers to administer the local anesthetic. This both saves you time and energy and allows him to accurately record the amount of local anesthetic that was administered for toxicity and documentation purposes.

Q: Can you sedate more than one patient in a day?
A: Yes. Many practices block a full sedation day and schedule several cases back to back.

Q: What if a patient isn't a candidate?
A: Dr. Schaake will screen the patient during the pre-op phone call with the patient and advise you if they are not a good candidate moderate IV sedation. Sometimes this involves recommending a dental anesthesiologist.

Q: How is moderate sedation different from deep sedation?
It is very different. Unlike deep sedation/general anesthesia, we do not guarantee the patient will be asleep for the procedure - although they will often drift off to sleep at times. They can be easily awakened verbally. Your patient will be comfortable and relaxed while still maintaining their own airway. In deep sedation, patients can lose competency of their airway, which requires intubation.

Q: Will my patient remember the procedure?
A: Yes, most likely they will remember parts of the procedure. This varies widely, with some patients remembering all of the procedure and others remembering almost nothing. However, we usually find that patients who remember the procedure remember it as being non-traumatic and positive. This is thanks to the beneficial effects of the opioid and benzodiazepine medications we use.

Q: Is moderate sedation really better than minimal sedation?
A: The effects are much more profound in moderate sedation. Minimal sedation only allows the use of one medication at a time, usually a benzodiazepine. Moderate sedation allows the use both sedative and pain-reducing (analgesic) medications which can be titrated to the optimal level for the patient based on the dosage required.

Q: Will you give my patient anything to help with pain?
A: Yes, if indicated and there are no medical contraindications, Dr. Schaake can administer IV Toradol and/or IV dexamethasone to help reduce post op pain and swelling from the procedure.

We have a one-hour case minimum of $800. Each additional hour is billed at $350 per hour.
